Spin the rings so only O−, O+, A− and A+ blood reaches the centre. That means straight lines linking T.F., H.M. and S.W. Then press the ORDELL button in the middle.
Where it is
It's in the Medical Center – Different, straight after the blood bags (Part 3, Chapter 8).
Why those three
Ordell is Simon's surname, and his blood type is on the Medical Wristband in your Equipment. It's what decides which blood types may reach the centre: O−, O+, A− and A+. The donors with those types are T.F., H.M. and S.W.
Wrong combinations just won't confirm. It can look like a bug, but it isn't. If you don't have the Medical Wristband, go back and find it rather than cycling through combinations.
After this, use the Room 204 Key (from the door dial) on the door at the end of the hallway. The next chapter starts right after the cutscene.
