The pharmacy locker code is 6539. Each poster in the pharmacy shows one number. Enter them in the order the CRTV broadcast shows the posters.
Where it is
It's the rusty locker in the pharmacy's back room in Shorefront District South (Part 1, Chapter 4). The pharmacy door is rusted shut, so use the Scalpel from the clinic on it.

Find the posters
Each poster in the pharmacy has a number on it. That gives you four digits.
Tune the CRTV to channel 30
The radio is on the back-room table. The broadcast shows the posters one at a time. That's the order you need.
Enter 6539
Inside the locker is the Prescription Dosage Error note, which you need for the clinic scales.
On PS5, CRTV tuning uses motion controls by default. If that's fiddly, turn off "Enable CRTV Motion Sensor controls" in the control options and tune with the sticks. The puzzle doesn't change.
